The Australian Transport Safety Bureau (ATSB) has confirmed the report that it is undertaking an investigation of an incident involving serious injuries of one seaman and the subsequent death of the second mate from the bulk carrier Hyundai Dangjin at Cape Lambert, Western Australia, that happened on 10 July.

The ship’s second mate was taking the draught marks when he fell into the sea from a rope ladder.  Other seaman raised the alarm from the deck before going for the rescue by jumping into the sea.  A lifeboat from the deck was also flung towards the second mate who was able to catch it and was unable to draw on.  This led to the death of second seaman whose deceased body was recovered later.  The other seaman swam back to the ship and was treated for hypothermia.
