AB Falls Into Unsecured Chain Locker During Mobilisation

While working on an anchor handling vessel in port, an Able Seaman (AB) accidentally fell into an open chain locker that was about 8 meters deep during preparations for pre-lay operations. Fortunately, the AB only suffered a minor head injury and was rescued within five minutes thanks to the onboard emergency procedures. He was taken to the hospital, treated, and was able to go home the next day, reports Marine Safety Forum.

Causes Identified

The investigation pointed out three main causes:

1. An unsecured hatch that was left open after previous operations.
2. Poor lighting in the chain locker area, as the overhead light was turned off.
3. Gaps in communication during operations involving the hatch.

Corrective Actions and Recommendations

1. Strengthen and formalise communication procedures with the bridge when operating hatches.
2. Make sure all openings are secured right after use.
3. Enhance lighting in operational areas and ensure there’s enough illumination before starting work.
4. Reinforce the STOP WORK authority for all personnel when they notice unsafe conditions.
