A bosun aboard a dredger suffered severe leg fractures while attempting to move a heavy sheave without proper planning or assistance. The incident highlights the critical need for careful preparation and teamwork when handling heavy equipment to prevent serious injuries, reports MAIB.
About the incident
In the early hours of the morning, a dredger was heading into port to discharge cargo and some equipment. As the crew prepared the vessel for berthing, the bosun and a deckhand discussed using the deck crane to offload equipment. Believing he had ample time, the bosun decided to prepare a 0.5-ton sheave for offloading. The sheave, stored vertically in a void space, was not secured properly. As the bosun attempted to maneuver the sheave past some oil drums, he lost control, causing the sheave to topple over and trap him underneath, inflicting severe injuries to his leg.
Despite being in considerable pain, the bosun managed to raise the alarm using his handheld radio. The crew quickly responded, freeing him from the sheave and providing first aid. Upon the dredger’s arrival at the berth, the bosun was transferred to a hospital, where he received treatment for multiple leg fractures. His injuries prevented him from returning to work for several months.
The Lessons
- Plan: The sheave was heavy, over 1 meter in diameter, and not stowed in an authorized location. The crew had no lifting plan in place to move it across the space. The bosun, unaware of its exact weight, underestimated the risks involved in its removal and relocation, leading to the accident.
- Teamwork: The incident underscores the importance of teamwork in safely completing tasks. While it may be tempting to work alone, especially in seemingly simple tasks, involving others can help identify potential risks, challenge unsafe plans, and provide support in case of emergencies. Proper teamwork can minimize risks and reduce the likelihood of serious injuries with long-lasting consequences.
