Transport Malta reports an incident involving enclosed space entry, leading to the loss of consciousness of three crew members aboard a motor tanker.
The Incident
On 15 May 2024, the motor tanker was engaged in bunkering operations off the Southeast coast of Malta. Shortly after the commencement of the bunkering operation, the chief engineer was found unconscious inside the starboard side slop tank. On instinct, the chief officer, followed by the master, went down the slop tank to assist the chief engineer, without the use of a breathing apparatus (BA) set.
Soon after, however, both the master and the chief officer were overcome by the atmosphere inside the slop tank, fell unconscious, and had to be helped out to the main deck. All three crew members were eventually rescued out of the slop tank and transferred to a local hospital, where they recovered several days later.
The MSIU has issued one recommendation to the Company designed to help safe entry into enclosed spaces.
Safety actions taken
During the safety investigation, the Company has carried out an internal investigation and its findings were shared among its fleet.
Recommendations
The company is recommended to: 03/2025_R1 ensure that crew members have a thorough understanding of entry and rescue procedures, carry out adequate maintenance and make proper use of personal protective equipment, communication equipment, and relevant instruments.
