Why It Happened?
The crew members were not secured to the vessel at the time of the incident. Safety harnesses were available but not used.
The ship’s low speed and adverse weather conditions further complicated rescue efforts.
Consequences
Despite extensive search and rescue efforts involving Norwegian helicopters, the missing crew members were not found.
The company identified deficiencies in securing mooring ropes and the use of safety gear.
Recommendations
To prevent similar incidents, the company issued safety circulars, supplied inflatable lifejackets.
They reviewed mooring rope storage and enhanced training on risk assessment and personal safety.
