Lessons Learned: Tow Strike on Bridge Caused by Operator Complacency

On April 23, 2023, an incident involving a towing vessel occurred on the Lower Mississippi River. The event was documented through an official investigation that analyzed the circumstances and causes of the contact between the vessel’s tow and a bridge structure.

What Happened

At approximately 22:42 local time, a towing vessel was pushing 25 barges downstream when the tow struck the center pier of a cantilever-style bridge crossing the river. One barge sank, and two others were damaged, resulting in an estimated $2 million in damages to barges and cargo. 

The towing vessel itself sustained no damage, and there were no injuries or pollution reported. The vessel’s captain had the watch as the tow rounded a river bend at 10.4 knots and intended to navigate through the recommended western channel beneath the bridge. 

However, the tow was significantly off course, and the captain attempted to steer through the eastern channel instead. Despite applying full engine power and rudder adjustments, the tow struck the bridge piers, and the tow subsequently broke apart.

Why it Happened

The incident was caused by the captain’s complacency, which led to inattention to the tow’s position approaching the bridge. Although the captain was experienced and had previously navigated this route many times, he acknowledged not paying sufficient attention and realized he was off his intended marks too late to correct the course safely. 

There were no indications of fatigue, impairment, distraction, adverse conditions, or mechanical issues contributing to the event. The repetitive nature of the task reduced cognitive effort and vigilance, increasing the risk of attention lapses.

Lessons Learned

Complacency due to repetition and monotony can impair situational awareness, even among highly experienced mariners. Maintaining focus requires deliberate strategies such as 

  • Continuous scanning of instruments and surroundings
  • Strict adherence to navigation procedures
  • Minimizing distractions
  • Changing physical position to maintain alertness (e.g., standing up or walking around)
  • Ensuring adequate rest and physical fitness

Implementing these measures can reduce the likelihood of attention lapses and improve safe navigation.
